Our CHNs help individuals understand their health options, book appointments, follow treatment plans, and overcome barriers such as language, fear, transportation, or digital literacy. But beyond these services, they also build trust—a critical but often overlooked factor in achieving long-term health outcomes.
By empowering CHNs with skills, mentorship, and resources, we not only strengthen community capacity but also reshape the relationship between public institutions and historically marginalized populations.
